Clinical Indication: Metformin and Vildagliptin are used together in type 2 diabetes because they have complementary mechanisms of action: Metformin improves insulin sensitivity and reduces hepatic glucose production. At the same time, vildagliptin inhibits DPP-4, enhancing incretin activity and increasing insulin secretion (and reducing glucagon) in a glucose-dependent manner, thereby improving overall glycemic control when either agent alone is insufficient.
